How to return your sample.

Instructions for sending the material for genetic testing by courier (TNT).

   Open the TNT shipment form

   Go to mytnt.tnt.com — create shipment and fill in the return form.

   Enter your pickup address

   Fill in your address details — the place the parcel is to be picked up from.

3. 3

   Enter the laboratory address

   Warsaw Genomics, Laboratory of Genetic Analysis, ul. Żwirki i Wigury 101, Bud. CNBCh, 02-089 Warsaw.

   Complete shipment & billing details

   If more than one test is ordered, please send the kits together. Choose "Receiver pays" and, in the TNT customer number field, enter the number shown in the instruction attached to the received collection kit.

   Choose a service and confirm

   Select "Show rates and times", pick your preferred pickup and delivery dates, then confirm with "Complete the shipment". You will receive an order confirmation with a return label by e-mail. If possible, print the label — otherwise the courier will have it.

How to properly pack the material for testing?

1. 1

   Mark the sample

   Label the test material with the patient's name, surname and PESEL number.

2. 2

   Secure the sample

   Wrap the material in a disposable tissue, paper towel or other absorbent material and put it in a plastic bag. Close the bag tightly and put it in a hard plastic wrap or small box. Put the package in an envelope (preferably bubble-wrap). Include the signed test order form and the patient's informed consent.

   Send the sample to the lab

   Send back the packed material according to the instructions above.

Step 1: Prepare the parcel

   Put the collected material into the cardboard box your kit came in, or pack it safely if you do not have our kit.

   Place the closed box inside the return packaging (poly mailer).

3. 3

   Seal the mailer tightly so the parcel does not open in transit.

Step 2: Create the shipment online

   Choose how to label the parcel

   Without a label or with a label.

   Choose a carrier and drop-off point

   InPost Parcel Locker, DPD Pickup or DPD parcel machine. Then click „Continue on the website".

   Sender details

   Enter your contact details.

   Confirmation

   Accept the terms (Easy Returns service and the carrier) and click „Create shipment".

Step 3: Drop off the parcel

• If you chose the option with a label — attach it to the parcel.
• If you chose the option without a label — write the code on the parcel.
• Drop the parcel off at your chosen point: an InPost Parcel Locker, a DPD Pickup point or a DPD parcel machine.
